Add questions to a request form
Ask for the details an approver needs to decide, so nobody has to chase them afterwards.
Before you get started
Account owners can manage request questions and request forms.
You need a request template to add questions to. See Create a request template.
Save the questions you'll reuse
Request questions are saved once and reused across templates. They're kept separately from performance review questions.
Go to Settings, then Request questions.
Create a question, choose its type, and add a description if the requester needs guidance.
Save.
You can edit, archive and restore saved questions from the same page.
Question types
Type | What the requester gives |
|---|---|
Text | A written answer |
Multiple choice | One option from a list you set |
5-point Likert scale | A rating from 1 to 5 |
Date | A date, such as a proposed start date |
Build the form
Open the request template, then select edit on the Request form card.
Add any instructions the requester should read first.
Add questions from your saved questions, or create a new one without leaving the form.
Mark the questions that must be answered as required.
Drag questions into the order you want.
Save.
Ask for what the final approver needs to say yes. For approval to hire, that's usually the role, the reason, the budget and a proposed start date.
What the requester sees
The questions appear in order when someone makes a request from this template. A request can't be sent until every required question is answered, and the answers are locked once it's sent.
Removing or changing a question later doesn't affect requests already made. Each request keeps its own copy of the questions it was asked.
