Emergency Contact form

The Emergency Contact form collects who to reach if something happens to someone at work.


Before you get started

Only account owners can create or configure paperwork templates.


What it collects

Always: the contact's name, relationship and phone number. Depending on how the template is configured, it can also collect an email, an address, and a secondary contact.


What can be configured

Open the template under Settings, then Templates, then Form templates, then Emergency Contact, and select Edit.

  • Default responsible person — who the form is assigned to for processing, if an employee self-assigns it.

  • Name — the label shown for this template. Defaults to Emergency Contact.

  • Email field — Required, Optional or Hidden.

  • Address field — Required, Optional or Hidden.

  • Secondary contact — Required, Optional or Hidden. When it is on, the person can add a second emergency contact alongside the first.

  • Instructions — optional text shown to the person filling in the form. Supports Markdown.
