Superannuation form
The Superannuation form collects someone's super fund choice, in the shape of the standard choice form.
Before you get started
Only account owners can create or configure paperwork templates.
What it collects
Nomination type (for example, Regulated), super fund name, USI, membership number, the person's name, and their tax file number.
What can be configured
Open the template under Settings, then Templates, then Form templates, then Superannuation, and select Edit.
Default responsible person — who the form is assigned to for processing, if an employee self-assigns it.
Name — the label shown for this template. Defaults to Superannuation.
Employer nominated super fund USI — the fund used if the person does not nominate their own.
Instructions — optional text shown to the person filling in the form. Supports Markdown.
