Tax File Number Declaration form

The Tax File Number Declaration form collects what payroll needs to withhold tax correctly for someone.

Before you get started

What it collects

TFN exemption, tax file number, name, address, email, date of birth, employment basis, residency status, HELP/VSL/FS/SSL/TSL debt, and whether to claim the tax-free threshold.

What can be configured

Open the template under Settings, then Templates, then Form templates, then Tax File Number Declaration, and select Edit.

• Default responsible person — who the form is assigned to for processing, if an employee self-assigns it.

• Name — the label shown for this template. Defaults to TFN Declaration.

• Instructions — optional text shown to the person filling in the form. Supports Markdown.

There is no per-field configuration on this template. Every field always appears.
