Questionnaire templates

Getting started - Create a new questionnaire template

Go to Settings > Templates > Add Template.

Select Questionnaire as the template type.

Only Account owners can create and manage certificate templates.

  1. Name – Enter a clear, obvious name.

  2. Instructions – Add helpful guidance to make sure questionnaires are filled in consistently or provide context as to why the questionnaire should be filled.

    • Instructions are optional and support Markdown formatting (paragraphs, lists, links, etc.).

  3. Questions – Choose which questions you wish to ask

  4. Click Add to save your template.


Configure your questionnaire template

  1. Open the created template in Worknice from the Templates area

  2. Add in your introduction text. This helps guide users

  3. Click Save when finished.

Adding a new question

Questions must first be created before they are added to a Questionnaire

  1. Go to Settings and Saved Questions

  2. Click Add Question.

  3. Enter a question label – this is the actual question the user will see (e.g., “How have you performed this year?”).

  4. (Optional) Add a Description – guidance text to provide context, link to external resources, or improve response consistency.

  5. Select a Question type – e.g., text, multiple choice, or scale.

  6. Click Add Question to save.


Editing an existing question

  1. Find the question you want to update.

  2. Click Edit.

  3. Update the label, description, or type as needed.

  4. Click Save.
